In January 2022, the journal Neurology1 published a report that should have awakened every neurologist in the country. The authors identified fragmentation as, “a major threat to neurologic care delivery,” and, “one of the most pressing challenges involving patient care.” The consequences for patients include higher costs, more hospital admissions, potential complications, unnecessary testing, and worse outcomes. They concluded that the solution to care fragmentation rely on:

  • Processes - Creating a patient and family centered team-based approach

  • Technology - Leveraging telehealth and electronic medical records
